The order entry form and material list is shipped with all Gordon-Piatt burners.  This form is EXTREMELY important and should be kept in a safe place.  The material list will help determine the correct repair parts for your burner.  Since Gordon-Piatt burners were "built to order" these forms give us valuable information that is not contained in your model number.
